Care Management -
Fee for Service
A Care Manager can evaluate the client's ability to live independently, with or without assistance. They can also assist the client and/or family in making arrangements for services such as personal care, homemaking, lawn care, doctor appointments, grocery shopping and selection and moving to an Assisted Living Facility or Nursing Home. The
Care Manager can also make regular visits to monitor the progress of the client and report back to a family member if so desired.
Case Management -
Grants
As lead agency, the Council on Aging provides Case Management and coordinates services for clients. An assessment is completed on each individual to determine functional impairment and risk of institutionalization, as is governed by the State. Services are then arranged with the appropriate provider.
Chore
Chore Services consists of minor household repairs and seasonal cleaning for eligible individuals. Simple household repairs are made to make the home safer.
Emergency Alert Response System
Arrangements can be made to install and
monitor, on a 24 hour per day/365 day per year basis, a push
button, voice system that, when activated by client, will summon emergency assistance.
Guardianship - Fee
for Service
Durable Power of Attorney
- A bonded, insured, experienced professional will work with the client and his/her
attorney to manage their financial, health and personal affairs. Pre-planning can ease
the burden for you and your family.
Financial
Counseling -
A
Financial Care Manager will take over the burden of balancing
checkbooks and bank statements, making deposits and writing checks
to pay bills for the client; checks are written at the direction of
the client and signed by the client. A Financial Care Manager can
also perform the cumbersome task of keeping up with Medicare and
supplemental insurance payments.
Professional Guardianship -
A Professional Guardian administers this service, which can be voluntary or court-ordered. Guardianship insures the professional control of all of the client's assets and
their daily living necessities.
Homemaking
Homemaking provides specific home management duties including light housekeeping, laundry, grocery shopping and other light household tasks.
Home Health Services
Home Health services can provide home health to those individuals under a doctor's order who require skilled medical services and are homebound. These services may be needed upon discharge from the hospital, nursing home or rehab center, or may be utilized to prevent health deterioration and hospitalization.
Medicare and private insurance accepted.
Customer Service
Information
concerning available services for senior adults is available through
the agency's Customer Service. If a service is not available through a Council on Aging program, a referral is made to other agencies which can best meet the client's needs. Customer
Service provides personal contacts with senior adults who need assistance with applying for services or encouragement to avail themselves of needed programs. More than 21,000 units of information are provided annually.
In-Home Services
An In-Home Care Aide will come to the home and assist with the activities of daily living. These activities may include: light housekeeping, personal care, shopping, laundry, respite and companionship.
Medical Transportation
Under certain circumstances, Council on Aging may contract with VOTRAN Public Transit System to provide Medical Transportation to doctors, rehabilitation, dialysis or similar treatments.

Meals on Wheels

MOW
are provided to homebound seniors over the age of 60. The meals are from a varied menu and provide one third of the daily nutritional
requirements, with no added salt, butter, seasons or sugar. A hot noontime
meal is delivered by volunteers Monday through Friday between the hours of 11a.m. and 1 p.m.
Congregate Dining

Congregate
Dining is Offered to seniors over the age of 60 in various at eight
dining sites located throughout the county, Monday - Friday.
Home
Ordered Meals

Home
Ordered Meals are available to anyone over the age of 18.
Arrangements can be made to receive a hot noontime meal, delivered
Monday - Friday directly to the client's residence.
Personal Care
Personal Care assists impaired elderly with bathing, dressing, grooming and walking.
Public Guardianship
COA is designated the Public Guardian for Volusia County by the
Statewide Public Guardian Office
. Operating under contract from the State of Florida and county
funding, indigent citizens who have no other willing or able family or friend to serve as guardian may be eligible for this program. The age criteria for this program is 18 years and above.
Respite
Care
In-Home
- Respite Care
relieves the caregiver from the constant and continued supervision of an impaired older person for a specific period of time.
Adult Day
Care - Adult Day Care provides a protective environment for
impaired persons who do not require institutional care. Individuals receive socialization and other activities in a
supervised setting.
Neighborhood Respite Program - Offers
caregivers a much needed break, while their loved one participates
in a therapeutic social activity program.
Grant services are partially funded by the State of Florida and/or Federal programs. Program participants must be 60 years of age or older and meet functional impairment levels to quality.
Services available under this program is solely dependent upon adequate funding from the State and Federal government. Due to the growing number of individuals in Volusia County over the age of 60 needing assistance, long waiting lists are not uncommon. |
