Council on Aging Home Health (COAHH) is a private not-for-profit home health agency. The mission is to promote optimal physical and emotional functioning through the provision of state of the art home health services by caring, competent and compassionate professionals.

Clients may qualify for Medicare Home Health:
Patients must be under the care of a physician who orders the home health care.
Patients must be homebound. Homebound means that it must be a definite hardship for the person to leave the home in order to qualify.
Patients must also require a skilled service to qualify. This means that the patient must need some type of skilled nursing care such as wound management, medication management, cardiac or respiratory monitoring, Diabetes management, physical or occupational therapy to assist in their recovery from conditions such as hip surgery or strokes.
If Clients do not qualify for Medicare:
COAHH can provide Home Health Aide services to assist patients with day to day living activities such as non-skilled medical care, bathing, light housekeeping and meal preparation.
COAHH can accept private insurance. Insurances differ in their requirements for patients to be homebound. COAHH staff will contact the insurance company to find out exactly what requirements must be met before admitting the patient into home care.
COAHH can work with the patients who are willing to self-pay for service. Also under the self-pay arena is private duty nursing. Some patients just want assistance with medications or assessments.
Services can be provided seven days a week, 365 days a year. Office hours are Monday through Friday, 8 a.m. - 5 p.m. To reach COAHH, call Council on Aging toll free at 888-252-6110, ext. 202, or email your question and a representative will contact you directly.
The Clinical Director for COAHH will be happy to talk with you about COAHH's services. After 5 p.m., during the week and on week-ends, COAHH has a Registered Nurse on call that can accept admission referrals or be available to patients receiving care who may have questions or an emergency.
So how do you go about getting Council on Aging Home Health services?
If your medical provider tells you that you need home health services, tell them that you want Council on Aging Home Health to provide the service. You, the patient, have the right to choose who provides your care. Once the referral is made, a Registered Nurse will come to your home and complete a detailed history and physical with you. They will develop a care plan with you and any family or caregiver you wish to include indicating how often you will be seen, what services are needed and who will provide them. They will also provide you with your rights and responsibilities including any financial obligations that you may have while you are receiving home health care.
COAHH communicates on a regular basis with your medical provider so that they know how you are progressing with your care. Most home health cases resolve within 45-60 days but sometimes last longer depending on the medical condition. Medicare patients must remain homebound throughout the home health episode or care will have to be discontinued.
